What is ADA 'UPLIFT'?
The Uplift Program is an American's with
Disabilities Act (ADA) service developed by MMVTA that provides
transportation to eligible candidates within the local
municipalities that are members of the Authority. Any Individual
with a disability who wishes to travel within the local service
area will be provided with origin to destination paratransit service if
eligible.
Cost
UPLIFT' fares are based on existing fare
structures for local routes. Due to the special nature of this
service, fares will double that of the fare for a comparable trip
on one of our local routes. UPLIFT will not accept half-fare or
senior citizen free cards or any current discount or special fare
programs. There is NO limitation to the amount of trips an
eligible individual can request.
Hours & Limits
If your point of origin is within one mile of
any of the Authority's local public transit routes between
California and New Eagle, UPLIFT will transport you, origin to
destination,
to any destination that is also within the stated one mile
limitation.
Uplift operates the on the following schedule:
Weekdays 6:20 AM - 11:25 PM
Saturdays 6:50 AM - 12:35 AM
Sundays 2:10 PM - 8:30 PM
Cards
Contact the Mid Mon Valley Transit
Authority to receive your application. Complete the forms
that pertain to you in their entirety and make sure that all
required signatures are obtained. When complete, return it
to the Mid Mon Valley Transit Authority. Your application
will then be reviewed to determine eligibility. One of four
different determinations will be made and you will be contacted
with the results via the U.S. Mail. These determinations are as
follows:
1.) Permanent Card
2.) Temporary Card
3.) Visitor Card
4.) Conditional Card
-Cards are valid for 3 years from issue date-
Trip Scheduling
Reservations for UPLIFT trips will be
taken up to the end of the proceeding business day. Trips to a
destination and from a destination are considered as 2 separate
trips. Cancellations must be done 1 day prior to your trip or it
will be considered as a no show. Several no shows could
constitute denial or suspension of future Uplift trips. The UPLIFT
service is permitted to be up to 20 minutes early or 20 minutes
late and will still be considered on time. You may bring a guest
with you on your trip but they will pay the same fare as the card
bearer. Personal Care Attendants (PCA)/Escorts may ride for free
if they are traveling to assist the card bearer. When applying for
an Uplift card, you must state that you are in need of a PCA/Escort
and it must be verified by a physician or agency. |
